Medications: *According to New York State Law* 1. A written prescription from a doctor is needed to dispense medication, including over the counter medications. 2. Parents written permission is needed to dispense medication. 3. The parent/guardian must bring the medication in. 4. The medication must be in the original bottle with students' name, drug name, dosage, route and time of administration on it. Students are not permitted to carry medication on them. The only exception is inhalers and there must be a doctor's order on file in the Health Office. Each school year new paperwork needs to be on file in the Health Office. Medications that are not picked up at the end of the year are disposed of. Injuries: Students may have injuries while they are at school, please encourage your child to report to the health office for any injury for the nurse to assess the injury. First aid will be given and if it is a serious injury a parent/guardian will be notified. Gym excuses: If your child can not participate in physical education he/she will require a doctor’s excuse. Please provide a copy to the nurse. Allergies: Please notify the health office of any life threatening allergies: For example: Peanuts, Tree nuts, Bee’s, Milk, Wheat, Egg, Soy or any other not mentioned.
