Mandated Screening, Physicals & Immunizations

Screening:

All screening will be done by the school nurse.

1. Scoliosis – grades 5-9

2. Vision – K, 1, 2,3,5,7 &10

3. Hearing - K, 1,3,5,7 & 10

4. Physicals – K, 2, 4, 7 &10

All new entrants will require all screening and a physical

Physicals:    

Grades K, 2, 4, 7, & 10 are mandated by New York State to have a physical. The physical can be done by the school physician Dr. Eppolito or by your personal physician. Check with the nurse to determine a schedule of dates when the physician will be at the school. If your child has a physical done by your own physician please provide a copy to the nurse as soon as possible. Physicals are good for one year.

Sports Physicals are offered at the school. Fall sports physicals are done in June. Please see the nurse for more info. The health office will offer a few sessions for spring sports, please check with the nurse for more details.

Any sports physicals that are done by your own physician need to be signed off by the school physician before the student can participate in a sport.

**All physicals need to go to the Health Office not the coaches**

Immunizations:

New York State requires the following:

DTP – 3 doses Hepatitis – All 7th graders must have 3 doses

Polio -3 doses Varicella (chicken pox) – 1 dose with a DOB after 1/1/94

MMR- 2 doses Tdap – All 6th graders are now required, as of 9/1/07, to have this booster.
